Location: Community-based across Central Lancashire – Chorley, South Ribble, and Greater Preston with some homeworking elements.
 
Hours: 28 per week

We are looking to welcome an approachable, enthusiastic, and knowledgeable, community-based Dementia Adviser to join our supportive and compassionate team, focusing on supporting people affected by dementia.
This role will have a focus on being a regular presence supporting outreach and group activities – Dementia Hubs, Dementia Café, Singing for the Brain – promoting engagement and wellbeing and being a trusted and friendly face to people looking for help.
This is a great opportunity to join our passionate and empathetic team providing personalised advice and support to people diagnosed with dementia and their carers and to promote our service and build relationships with a range of health and social care professionals in the voluntary and private sector
You will become part of dementia provision and third sector partnerships in Central Lancashire.
What you will do;
Offering a vital, compassionate advice service to support and guide those affected by dementia - in person and by phone.
Provide community based, face to face for an increased level of support for more complex cases.
Managing referrals, assisting clients in assessing their information and support needs in a person-centred manner.
Problem solving – helping people find ways to better live around symptoms and challenges that come from their dementia.
Providing knowledgeable and trusted information regarding services and benefits available and, where appropriate, assist clients in accessing them.
Facilitating group services for people with dementia and for carers.
Networking local services to strengthen a Dementia Friendly Community.
Developing a proactive approach in reaching people with dementia and carers who may not otherwise access our services.About you
A strong understanding of dementia.
Good organisational and time management skills.
An ability to assess client needs along with the understanding of the need for client confidentially.
Strong communication skills, both written and verbal.
Good organisation and time management skills.
An ability to manage and support challenging emotional situations.
Strong ICT skills.
