✓Role Overview – Health & Wellbeing Practitioner:
✓As a Health and Wellbeing Practitioner, you will be responsible for delivering client-centred treatment based on individual requirements
✓Managing a caseload, you will provide appropriate harm reduction advice, assess, and create care plans
✓This role involves conducting structured 1:1 and group-work sessions for Substance Misuse and Low-Level Mental Health service users
✓Additionally, you will support transitional services for individuals transitioning from prison to the community, working collaboratively with other Health and Wellbeing agencies to prepare service users for release.
✓Responsibilities – Health & Wellbeing Practitioner:
✓Develop and contribute to the Substance Misuse and general Health and Wellbeing care needs of service users, supporting them in the development of release plans which are appropriate to their needs.
✓Ensure substance misuse and mental health groups, and Health and Wellbeing programmes, are facilitated/co-facilitated to the standard required by the Forward and in accordance with any relevant needs.
✓Carry out substance misuse and low-level mental interventions relevant to individual client needs, including 1:1 sessions and group work/programme facilitation.
✓Requirements – Health & Wellbeing Practitioner:
✓Knowledge of the challenges faced by substance misusers and individuals with low-level mental health issues.
✓Demonstrable experience in conducting risk assessments, comprehensive assessments, and designing/implementing SMART care plans.
✓Significant experience in delivering psychosocial interventions, including the use of motivational interviewing techniques, in both 1:1 and therapeutic group settings.
✓A strong understanding of and commitment to the 12-step process of recovery from addiction.
✓Previous experience in providing counselling/CBT services to prisoners, individuals with Health and Wellbeing concerns, or those dealing with drug and/or alcohol-related issues Show more →
