Hours – 35 hours per week [+ 5 hour paid lunch]

[hybrid office work / working at the office a min of 2 days a week]

Who we are looking for?
We are searching for a Senior Project Worker in Wales to provide housing and property management related support to vulnerable individuals together with the aim of achieving a successful move-on and other measurable outcomes.
This role includes managing a small team of project workers and also carrying a small caseload of service users, alongside directly supporting own case load of Service Users.
It is essential to have experience of working with people who have complex needs, to have a good understanding of safeguarding processes and procedures.
Knowledge of housing legislation, experience of partnership working and experience of leading a team would be advantageous.
A diploma or certificate in Housing or Health and Social care is desirable.
Duties and responsibilities include but are not limited to:
· Engaging with referral agencies, such as local authority, access point/gateway services, statutory and voluntary sector agencies in order to generate referrals for the service as required.
· Carry out initial assessments for service users including any risks of their requirements to determine eligibility.
· Designing and delivering effective support plans, based on the service user’s needs and abilities.
· Maintain contact with service users as required depending on their needs maintaining a professional relationship and becoming a role model.
· Ensuring that where required housing and property management tasks are effectively undertaken in line with procedure and targets.
A DBS check will be required for this role.
A full driving licence and own vehicle is required to be successful in this role
