✓Role Overview – Team Leader:
✓As a Team Leader within our Substance Misuse Services, you will play a vital role in leading a team of Health and Wellbeing Practitioners
✓You will be responsible for achieving targets set by Forward and local commissioners, providing supervision, training, and guidance to practitioners, and supporting the unit's management team
✓This position requires a strong focus on measuring outcomes, emphasizing the importance of collaborative relationships with partner agencies.
✓Responsibilities – Team Leader:
✓Support the service manager in developing relations and set team and individual objectives to implement and achieve strategic objectives.
✓Take an active role in the implementation and achievement of team objectives and developing supportive relationships with supervisees and other colleagues in the unit
✓Alert the Service manager to any significant risks or problems arising in the course of managing and monitoring the services.
✓Ensure case management/programme documentation is maintained to the expected audit standards.
✓Support and monitor supervisees to manage their caseload effectively, making sure that clients are prioritised appropriately and seen in a timely manner.
✓Ensure that case management interventions are delivered in accordance with the needs of the service users, audit and are CQC compliant.
✓Qualifications and Experience – Team Leader:
✓Proven experience working within substance misuse services within a prison setting.
✓Demonstrated ability in direct line management, including supervision, appraisals, guidance, and constructive feedback to supervisees.
✓Experience conducting comprehensive assessments, designing SMART care plans, and implementing them effectively.
✓Proficiency in delivering structured interventions to service users and facilitating therapeutic groups.
✓Previous involvement in a 12-step primary care program and utilization of clinical supervision.
