Role Overview – Team Leader:
As a Team Leader within our Substance Misuse Services, you will play a vital role in leading a team of Health and Wellbeing Practitioners
You will be responsible for achieving targets set by Forward and local commissioners, providing supervision, training, and guidance to practitioners, and supporting the unit's management team
This position requires a strong focus on measuring outcomes, emphasizing the importance of collaborative relationships with partner agencies.
Responsibilities – Team Leader:
Support the service manager in developing relations and set team and individual objectives to implement and achieve strategic objectives.
Take an active role in the implementation and achievement of team objectives and developing supportive relationships with supervisees and other colleagues in the unit
Alert the Service manager to any significant risks or problems arising in the course of managing and monitoring the services.
Ensure case management/programme documentation is maintained to the expected audit standards.
Support and monitor supervisees to manage their caseload effectively, making sure that clients are prioritised appropriately and seen in a timely manner. 
Ensure that case management interventions are delivered in accordance with the needs of the service users, audit and are CQC compliant.
Qualifications and Experience – Team Leader:
Proven experience working within substance misuse services within a prison setting.
Demonstrated ability in direct line management, including supervision, appraisals, guidance, and constructive feedback to supervisees.
Experience conducting comprehensive assessments, designing SMART care plans, and implementing them effectively.
Proficiency in delivering structured interventions to service users and facilitating therapeutic groups.
Previous involvement in a 12-step primary care program and utilization of clinical supervision.
