As an Adults Care Manager in the Hospital Team, you will play a crucial role in ensuring the provision of high-quality social work services to individuals receiving care in hospitals
You will lead a team of dedicated social workers and collaborate with multidisciplinary professionals to promote positive outcomes for adults and their families.
Responsibilities:
Manage and oversee a team of social workers, providing guidance, support, and supervision to ensure the delivery of high-quality care and support services.
Conduct comprehensive assessments of adults in hospitals, identifying their needs and risks, and developing appropriate care plans in line with legislation and local policies.
Coordinate and facilitate effective discharge planning, ensuring smooth transitions from hospital to community-based services.
Collaborate with health professionals, including doctors, nurses, and therapists, to provide holistic care and support to individuals.
Ensure compliance with relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005, while safeguarding the rights and welfare of adults.
Monitor and review care plans, ensuring they are person-centred, regularly evaluated, and updated to meet changing needs.
Provide expert advice and consultation to the team and other stakeholders on complex cases and issues related to adult social care in a hospital setting.
Promote effective communication and partnership working with internal teams, external agencies, and service providers to ensure seamless delivery of care services.
Contribute to the continuous improvement of service provision through participation in quality assurance activities, case reviews, and professional development initiatives
Requirements:
Qualified Social Worker with relevant registration [SWE] and professional qualification [Essential]
Extensive experience in adult social work, including working with individuals in a hospital or healthcare setting.
Strong knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and associated statutory frameworks
