✓Overview of a Support Coordinator
✓Pertemps have an exciting opportunity within Harrow Council for a self-driven and motivated Support Coordinator to join an expanding team
✓The role of a Support Coordinator has a competitive pay rate of £15.68 - £16.26 Per Hour
✓Other benefits include excellent training and support for career development and 21 days holiday [excluding Bank Holidays]
✓The Main Purpose of a Support Coordinator:
✓"To comply with all Sheltered Housing policies and procedures, including those for Lone Working, Home Visits for Sheltered Housing Applicants and Support Delivery Responsibility for undertaking any revision to entitlement/contribution for activ-ities.
✓To effectively manage a caseload and case closure.
✓To deal effectively with a range of complex support needs and risks that have sig-nificant implications for Tenants and prospective Tenants.
✓To monitor the well -being of Tenants through daily or weekly contact as agreed.
✓To participate in a weekly rota to provide on -site support surgeries at each scheme.
✓To participate in a rota to undertake needs and risk assessments for prospective Tenants
✓These assessments are undertaken in applicant's homes.About you, the successful Support Coordinator will have:
✓Knowledge and understanding of Health and Safety requirements of Sheltered Housing Schemes
✓Ability to organise and prioritise workload and act promptly in urgent situations.
✓Ability to communicate and liaise effectively with colleagues, Tenants & their carers, other Council Departments and external agencies.
✓Significant experience in a similar role working with older people.
✓Knowledge and understanding of Health and Safety requirements of Sheltered Housing Schemes
✓Ability to work outside office hours occasionally and to attend occasional evening meetings when required.
