✓This particular project will be supporting a 21 year old female, providing a support to promote independence.
✓The Model of Enhanced Supported Living has been carefully developed to support adults living with enduring mental health problems, learning disabilities and autism 24/7
✓We support people with long term mental health conditions, who require access to professional clinical and social support to enable them to reach their goals
✓Along their journey to mental health recovery, outcomes are consistently measured, involving both the individual and their wider support network [clinical, social, economic and qualitative].
✓The Enhanced Supported Living Model aims to Support people with long term mental health conditions to become more independent and achieve their goals in a safe, comfortable living environment.
